The short version: an adapter changes the shape of the plug. A converter changes the voltage. Almost all modern phone, laptop and camera chargers are dual-voltage and need only an adapter. Heat-producing appliances — hairdryers, straighteners, travel kettles — are the ones that need either a converter or a dual-voltage model.

How to tell whether you need a converter
Read the small print on the charger or the appliance itself. You are looking for the input range:

INPUT: 100–240V, 50/60Hz— dual voltage. You only need a plug adapter. This covers nearly every phone charger, laptop brick, camera charger and electric toothbrush made in the last fifteen years. -
INPUT: 120V only— single voltage. Plugging this into 220–240V without a converter will destroy it, sometimes dramatically. Common on cheap hairdryers, curling irons and some kitchen gadgets.
